This public website is not intended for the submission of medical records, protected health information, or detailed case information. A website inquiry is not the medical intake process.

Information We Do Not Ask You to Submit Through the Public Website
The public inquiry forms are not designed to collect:
- Medical records, diagnoses, or detailed medical histories
- Social Security numbers, VA file numbers, or dates of birth
- Disability ratings
- Client medical records or sensitive case descriptions
- Record uploads or other files
Please do not place this information in ordinary website or email communications unless Nexus provides appropriate instructions. Although the forms are not designed to request such information, Nexus cannot guarantee that unsolicited information will never be sent.

Website and Technical Information

Nexus does not use advertising cookies or behavioral advertising on this website. Optional Google Analytics operates only after affirmative visitor consent as described below. Strictly necessary browser or hosting technologies may still be used to deliver website functions.

Nexus uses optional Google Analytics to understand how the website is used and improve the experience. Analytics is off unless you affirmatively allow it. You may decline Analytics and continue to use the website.
When allowed, Google Analytics may set or read Analytics cookies and process limited website usage and technical information, including sanitized page paths and standard browser, device, and network information. Google acts as the analytics service provider.
Nexus does not send inquiry names, email addresses, phone numbers, firm names, medical records, medical details, case information, or form contents to Google Analytics. Nexus does not use a user ID or hashed identifier for this measurement.
You can change your choice later with the Analytics preferences control in the website footer. If you turn Analytics off after allowing it, Nexus stops further Analytics events from this website and attempts to remove Analytics cookies accessible from the current website origin. Browser restrictions may prevent website code from removing cookies that are unavailable to JavaScript or were created under a different domain or context.

Medical Records and the Separate Intake Process
Submitting a public inquiry is not the medical intake. If Nexus determines that a medical review may proceed, Nexus will provide separate instructions for submitting case information and records through the appropriate intake process.
Do not send medical records or sensitive medical information through the public inquiry forms or ordinary email unless Nexus specifically instructs you otherwise. Not every inquiry will proceed to medical review.

How Long Information May Be Processed or Retained
The Nexus public website does not maintain a dedicated database of inquiry submissions. Inquiry information is transmitted through Nexus's transactional email workflow and may remain in Nexus business email systems and service-provider systems for operational, recordkeeping, security, legal, or business purposes.
Retention by service providers may be governed by their own policies and agreements. Nexus will retain information only for as long as reasonably needed for legitimate business or legal purposes, subject to final policy and legal review.

The website and Nexus services are not directed to children under 13. Nexus does not knowingly use the public inquiry forms to solicit personal information from children under 13. If Nexus learns that such information has been submitted, it may take appropriate steps to delete or otherwise address it.
